To request a GP medical report for a Firearms Licence or Shotgun Certificate, please complete the form at the bottom of this page and submit to the Surgery.

The production of these reports falls outside of our GMS Contract and is not part of our core GP service.  In the past this has been done free of charge as a goodwill gesture, but given the increasing number of requests we are receiving and the amount of time these take to complete we regret we can no longer continue to produce these for free.  From the 1st January 2021 a fee of £25 will be charged for the production of all medical reports relating to either a new application or for the renewal of a firearms/shotgun licence.

Please ensure you include a mobile telephone number or e-mail address when completing your details.  Once the report is complete, we will send you a link via Sumup so that you can make payment by either debit or credit card.  Once payment is received, we will forward the report to you.

Please note it can take up to 21 days for the report to be completed.
